A extremely rare case of gastrobronchial fistula is reported with brief review of literatures. A 68-year-old Korean woman entered the Severance Hospital, Yonsei University College of Medicine, because of sudden onset of chills, high fever and cough with profuse yellowish-green sputum of three days duration. For the past 30 years after laparatomy, she has had persistant mild productive cough. On physical examination, moist rales was audible on the right lower lung field. Peripheral blood showed leukocytosis, and sputum culture yieded Staphylococci and E. coli. Chest X-ray showed large ill-defined homogeneous, density in the right lower lung. The patient was treated with antibiotics under the diagnosis of lung bscecc satisfactorily. Upper gastrointestinal series demonstrated a fistulous communication between the antrum of the, stomach and the mid lobe of the right lung. Gastrobronchial fistula was confirmed by abdominal exploration and fistulectomy with pyloroplasty was performed succesfully. Since the, operation she has been carrying on normal life without any problem.
